Rabies in Thailand: What to Do After a Dog or Monkey Bite

Thailand still records 100+ human rabies deaths every year, almost all from stray dogs and macaque monkeys. Rabies has a 100% fatality rate once symptoms appear. But post-exposure prophylaxis is essentially 100% effective if you start within 24 hours. Do not 'wait and see'.
Symptoms to watch for
- •Any bite, scratch, or lick on broken skin from a dog, cat, monkey, bat, or wild animal
- •Initial: tingling at wound site, fever, headache (days to weeks later)
- •Later: anxiety, confusion, hyperactivity, hydrophobia (fear of water), paralysis
- •Once symptoms start, death is certain within 7–14 days
DO
- ✓Wash the wound IMMEDIATELY with soap and running water for 15 minutes
- ✓Apply povidone-iodine or 70% alcohol
- ✓Go to any Thai hospital or Saovabha Institute (Bangkok) within 24 hours
- ✓Get rabies vaccine series (5 doses over 28 days) + immunoglobulin for category III exposure
- ✓Photograph the animal if safe — does not change treatment but helps authorities
- ✓Report bite to local authorities if from a stray
DON'T
- ✗Do NOT wait to see if the animal seems sick — by then it is too late
- ✗Do NOT sew or close the wound — it traps virus
- ✗Do NOT skip doses of the vaccine series
- ✗Do NOT pet stray dogs, monkeys, or visit monkey temples without awareness
- ✗Do NOT assume vaccinated pets in Thailand are safe — many vaccination records are unreliable
When to go to the ER
Any animal bite or scratch in Thailand is an ER visit within 24 hours. Saovabha Institute in Bangkok and most provincial hospitals stock vaccine and immunoglobulin. Cost ~3,000–8,000 THB if not covered by insurance.
