The stonefish (Synanceia) is the most venomous fish on Earth and present along Thailand's Andaman and Gulf coasts. It looks exactly like an algae-covered rock and is invisible until you step on it. The pain is so extreme victims have been described as 'wanting the limb amputated'. Hot water is the only effective first-aid.

Symptoms to watch for

  • Immediate, excruciating pain — often described as the worst pain imaginable
  • Rapid swelling, blue-gray discoloration around puncture
  • Nausea, vomiting, sweating, dizziness
  • Difficulty breathing, low blood pressure
  • Paralysis of affected limb, possible cardiac arrhythmia
  • Death in untreated severe cases (rare but documented)

DO

  • Get out of the water — drowning risk from pain shock is real
  • Immerse the wound in HOT water (~45°C, as hot as the victim can bear) for 30–90 minutes
  • Remove visible spine fragments with tweezers
  • Call 1669 — stonefish antivenom exists and many Thai coastal hospitals stock it
  • Update tetanus vaccine
  • Get an X-ray to confirm no spine fragments remain

DON'T

  • Do NOT apply ice — concentrates venom and increases tissue damage
  • Do NOT use a tourniquet
  • Do NOT cut or suck
  • Do NOT walk barefoot in shallow water on rocky/reef areas
  • Do NOT delay — the hot water window is most effective within the first hour
